NOTE: Participants should complete their evaluation submissions within 30 days of the end of the meeting.

Instructions

Enter your E-mail address and your password. If the system does not recognize you, please create an account for yourself.

You may SAVE your file and return to it at any time to add more sessions or make changes up to 30 days after the meeting ends.

When you have completed evaluations for all the sessions you attended, click on the FINISHED button. At this point you will not be able to make further changes to your CME record and you can either print out your CME certificate or have it e-mailed to you at your request.

You may log back into the system at any time to print out a duplicate copy of your certificate using your E-mail and last name.

Contact us if you have any issues with the system.

Certificate Printing Problems

If you cannot get your certificate to print out, it might be due to pop-up blockers on your browser. You will need to change your settings to allow pop-ups for this site. To do so, please follow these instructions (using the menu bar at the top of the browser):

Windows browsers:

  1. Internet Explorer: Go to Tools>Pop-up Blocker>Pop-up Blocker Settings. In the ensuing window, type “dspesg.com” into the field at the top and click Add.
  2. Firefox: Tools>Options>Block Popup Windows. Make sure this option is UNCHECKED or you can select the option to add &ldquoldspesg.com” as an allowed site.
  3. Netscape: Go to Tools>Pop-up Manager>Manage Pop-ups. In the ensuing window, type “dspesg.com” and click Add.

Mac OS X browsers:

  1. Safari: Go to Safari>Block Pop-Up Windows. Make sure this option is UNCHECKED.
  2. Firefox: Go to Firefox>Preferences>Make sure this option is UNCHECKED or you can select the option to add “dspesg.com” as an allowed site.
  3. Netscape: Go to Netscape>Preferences. A new window will open. Single-click &lqduo;Privacy & Security,” then single-click “Popup Windows.” Make sure “Block unrequested popup windows&rdquol is UNCHECKED — OR leave it checked and single-click “Allowed Sites&hellip.” Then type “dspesg.com” and click “Add.”
  4. Internet Explorer: Does not block pop-ups, so there shouldn’t be a problem.

  1. Why can’t I log in?
    • Make sure you have the correct…
      E-mail address
      If someone else registered you for this Web site, try their e-mail address instead.
      Password (at least four characters in length)
      If you have forgotten your password, click on the “forgot your password” link at the bottom of the page. Then enter the e-mail address you used when you registered for the conference and click the “Submit” button. Your password will be sent to that e-mail address.
    • This system requires self-registration. If you have not created an account yet, please click on the “create an account” link at the bottom of the page.
  2. Why does the ASH Web site evaluations system refuses to recognize my e-mail address?
    • If someone else registered you for the conference, try their e-mail address.
    • If you have multiple e-mail addresses, try another that may been used to register you.
    • If all else fails, click the “Contact” link at the top right of the page to contact Web site support.
  3. I requested that the system email me my certificate, but I never got it…
    • Try again, and make sure the email address in the window is spelled correctly and is the email address currently in use by you.
  4. I was never mailed my Certificate…
    • The System provides certificates by email or you can print it by selecting print. Please log on to the website to receive your certificates for the sessions you have evaluated in this manner.
    • If more than 30 days have expired since the end of the conference and you have not already completed your evaluations, you will have to contact the ASH Staff at ash@ash-us.org and ask them to authorize us to unlock your records so that you may be provided “Late Entry” evaluations for CE Credit.
  5. I see a session I attended but there is no way to select it…
    • The system monitors the sessions that you select and prevents you from selecting multiple sessions with conflicting time periods. If the session time periods conflict, and you would prefer credit for a different meeting, you must click the contact icon at the top right of the window and then complete the form with your request for us to de-select the session selected by mistake. We will let you know when this is accomplished.
  6. The window to access the meeting has closed and I would like late entry into the system…
    • First make sure you have created an account. If not, please do so. Then contact ASH Staff directly at ash@ash-us.org and request late entry into the meeting. They will then authorize us to unlock your records so that you may provide “Late Entry” evaluations for CE Credit.
  7. I missed the deadline for submitting my evaluations; what can I do?
    • Contact ASH Staff at ash@ash-us.org and ask them to authorize us to unlock your records so that you may provide “Late Entry” evaluations for CE Credit. Once authorized, we will be glad to allow you to enter your evaluations for CE Credit.
  8. I hit finished before I completed all of my evaluations; what should I do?
    • Contact ASH Staff at ash@ash-us.org and ask them to authorize us to unlock your records so that you may provide “Late Entry” evaluations for CE Credit. Once authorized, we will be glad to allow you to enter your evaluations for CE Credit.
